An application for a graduate assistantship may be obtained in any graduate department or in the Graduate School, or it may be downloaded from the links below. An HTML version is included for your information but may not be acceptable to your department. When filled out, the form should be submitted to the department of the student's chosen graduate major. Both the WP version and the MSWord version are intended to print on both sides of one sheet of paper.
